How to Register as a New Patient

Welcome! We invite all new patients to join Abbeywell Surgery. You may be seen at either of our two Romsey-based sites – Abbey Mead and Nightingale.

To register as a new patient, you will need to live within our practice boundary – feel free to use our boundary checker below before starting the registration process.

To register with us, please complete our online registration form. Please ensure that all details are completed, as this can cause delays in being register

Note that to complete the form you will need to upload pictures of 2 forms of identification. Photo and proof of address

Proof of identity e.g. photo drivers licence or passport

Proof of address e.g. recent utility bill or bank statement

After registering with the practice, if you are on repeat medication, you may be asked to make an appointment at your convenience to see your new doctor or one of the practice nurses for a brief health check.
