Summary Care Record (SCR)
A Summary Care Record is an electronic record of your healthcare history held on a national healthcare records database provided and facilitated by NHS England. This record may be shared with other healthcare professionals and organisations involved in your direct healthcare. It gives healthcare staff faster, easier access to essential information about you, to help provide you with safe treatment when you need care in an emergency or when your GP practice is closed.
At a minimum, the SCR holds important information about;
- Current medication
- Allergies and details of any previous bad reactions to medicines
- The name, address, date of birth and NHS number of the patient
You can choose to have a Summary Care Record.
You do not need to do anything. This will happen automatically. Healthcare staff will ask your permission every time they look at your Summary Care Record.
You can choose not to have a Summary Care Record.
If you don’t want a Summary Care Record, you need to let us know by filling in the Abbeywell Surgery SCR Opt Out Form.
You can change your choice at any time.
Privacy & Data Protection
Patients and staff have the right to be treated with respect and dignity, so we maintain high standards of confidentiality for Patients. The Practice is bound by the Data Protection Act 2018 which governs the storage and processing of personal data held on computers. Patients have the right to access their computer and manual records.

Statement of Purpose:
Under the Health and Social Care Act 2008 every registered provider must have a statement of purpose.
Our Statement of Purpose which includes a standard required set of information about our service and describes:
- The provider’s aims and objectives in providing the service.
- The kinds of service provided.
- The health or care needs the service sets out to meet.
- The location where the service is provided from.
- Details from the provider, including their legal status, manager including the “address for service” for all registered persons.

Freedom of Information
The Freedom of Information Act 2000 aims to produce a culture of openness in public bodies, by providing members of the public with the right to access information held. The main purpose behind this is to show how public money is being used.

Practice Complaints Procedure
We strive to deliver an excellent standard of care to all of our patients but if we have fallen short of what you expect, please let us know. The practice operates a Complaints Procedure as part of the NHS system for dealing with complaints. Our complaints system meets national criteria.
Any complaint is treated in confidence and will not have an impact on your care or treatment. If you make a complaint, it is practice policy to ensure you are not discriminated against and there will be no negative effect on your care, treatment or support as a result.

Zero Tolerance
Abbeywell Surgery supports the NHS Zero Tolerance Campaign. People who show signs of violence to Patients, Staff or persons present on Practice premises will be reported to the Police. Patients who do this will also be removed from the Practice list.
Named GP
We have allocated a Named Accountable GP for all of our registered patients. New patients joining us will be advised of their Accountable GP at the point of registration. If you do not know who your named GP is, please ask a member of our reception team.
GP Earnings Disclosure
NHS England require that the net earnings of doctors engaged in the practice is publicised by 31 March 2024 at the latest. However, it should be noted that the prescribed method of calculating earnings is potentially misleading because it takes no account of how much time doctors spend working in the practice and should not be used for any judgement about GP earnings, nor to make any comparisons with other practices.
The average pay for GPs working in Abbeywell Surgery in the last financial year was £115,252 before tax and National Insurance. This is for 4 full time GPs and 6 part time GPs who worked in the practice for six months or more.
